Investigation Nr: 170593057
Event: 08/04/1994
EMPLOYEE INJURED HEAD IN HEAT SEALING PRESS MACHINE

At approximately 6:50 a.m., on August 4, 1994, an employee was operating a heat sealing press machine, forming and sealing medical urine bags. To correct a problem with a production run, the employee reached and ducked underneath the plastic sheet puller device to re-tape the plastic ends. The employee accidentally actuated the machine and the hydraulic piston pulling arm cycled, pinching his head sideways between the piston arm and the upper frame. He suffered contusions and bruises, and was hospitalized.
