Investigation Summary
At about 8:30 a.m. on March 29, 1994, Employee #1 was preparing a larger pipe for installation of a smaller pipe collar. Employee #1 drilled a hole in the larger pipe and filed the surface around the drilled area. After that, the collar was prepared for welding. When the welding arc was struck, there was a petroleum vapor explosion. Employee #1 suffered a broken hip, for which he was hospitalized.
