Investigation Summary
At 7:30 p.m. on July 8, 1999, Employee #1 noticed that the 0.219-inch diameter rod on the WEB-MORE MORGAN SERIES Block Number 7 was running unsanded. Upon checking, he observed that the sanding belt had broken. Employee #1 pressed a stop button on the machine to stop it. While the machine was slowing down, Employee #1 walked back to the sanding unit. The wire drawing machine stopped and Employee #1 opened the center door of the sanding unit. Employee #1 reached into the unit to rotate the flyer assembly to insert the extension tool to release the tension on the sanding belt rollers. In the absence of the sanding belt on the rollers, there was no friction to quickly slow the spinning rotation of the belt pulleys, therefore the driving mechanism was freewheeling. When Employee #1's hand came in contact with the timing belt behind the flyer, it pulled his gloved left hand in and around one of the timing belt rollers, causing him to lose the tip of his left index finger. The free wheeling belt and pulley drive mechanism did not completely come to a stop prior to his inserting the extension tool between the pulleys. Employee #1 was taken to the Concentra Occupational Medical Center in Ontario, California. Employee #1 was hospitalized for surgery that was performed at the Chino Valley Hospital in Chino, California.
