Employee falls through floor opening

At approximately 11:00 a.m. on July 16, 1999, Employee #1 was in the process of installing blocks 8-in. by 8-in. by 2-in. for clean out work while sitting on an edge of an unguarded triangular floor opening of 68 inches long by 26 inches wide. He fell 16.67 ft to the concrete floor. The floor opening was not guarded by a floor covering or by signs. Employee #1 received multiple fractures to his right leg below the knee and right arm above the elbow, dislocated right shoulder, broken nose and an abrasion to the left side of his face. He was hospitalized.
