Employee Suffers Head Injuries

At approximately 3:30 p.m. on April 14, 1999, Employee #1 and a coworker were bolting and welding prefabricated steel columns and I-beams in a garden center that was under construction. The employees were working on column line Number three at a height of 15 ft 3 in., from a fully-extended elevated platform (M19 - Scissor Lift Serial Number 8531). After finishing work at column line number three, the employees were riding the moving scissor lift with a fully extended platform. A pile of debris on the concrete floor made the floor uneven. During travel, the right front wheel of the scissor lift rolled over the pile of debris causing the scissor lift to tip over. Employee #1 sustained serious head trauma injuries and the coworker sustained only minor injuries. Employee #1 was hospitalized.
