Investigation Summary
Employee #1, who worked for an employment agency, had been sent to a company that manufactured health products, such as vitamins. He was working on line #12, at a PDC banding machine that cuts plastic rings and seals them onto the caps of bottles. Employee #1 and coworkers were setting up the machine when it became jammed. Employee #1 was reaching into the machine from the back to clear the jam when the blade of the cutter became free, amputating the tip of his finger.
