Employee's fingertip crushed by machine plunger

On the morning of September 22, 1995, Employee # 1 was about to start work in the tube straightener. He turned on the machine with his right hand using a wall-mounted switch, while his left hand was between the plunger and anvil (where tube is positioned for straightening). Employee #1 did not see that the machine had not completed its cycle when turned off after the previous shift. The plunger smashed his fingertip against the anvil. Employee #1 was treated at the Gateway Medical Center, La Mirada. Causal factors include: the inadequate design of the machine, which was manufactured in-house; the fact that it could become operational and start plunger motion by a single energy source (the wall switch); the fact that the machine had not completed its cycle from the previous shift; and the fact that the activating lever was in the ON position.
