Investigation Summary
At 10:00 a.m. on September 23, 1995, Employee #1 was operating a precision adjustable stroke boring machine with moving, horizontally opposed, 1 in. diameter boring bits. The bit motion is activated by a flip lever for each bit. The point of operation was left unguarded for hand access. Employee #1 had been working the second shift for over six hours when he dropped a metal chip cleaning tool. While he reached for it, his arm entered the path of the boring bit and was penetrated. Employee #1 called for help but the rescue inadvertently made the bit penetrate deeper. Employee #1 was taken to the hospital, where he was treated and released for drainage to occur before the eventual surgery. There were no witnesses to the accident. Cause of the injury was the lack of a guard over the point of operation. An accident related violation was cited: serious T8CCR 4184(b).
