Employee's finger amputated in stitching machine

Employee #1, of the temporary agency, Staff Control, had been assigned to work at ACX. He was operating a bag stitching machine, a device used to fill 50 lb sacks with hay cubes and then sew them shut. Employee #1 was overseeing the sewing phase when his little finger became caught in the 1/2 in. opening that allows burlap sacks to enter the point of operation. His finger slipped past the hand guard and was partially amputated in the machine. The guard mechanism was inadequate, and Employee #1 had not been properly trained on this machine. He was a temporary worker and had been given this assignment without his employer's knowledge.
