EMPLOYEE INJURED WHEN THUMB CRUSHED BY SHIFTING LOAD

At 9:00 a.m. on July 11, 1994, Employee #1 was moving a 4-inch by 4-inch block of wood under an elevated load of a forklift truck. The load came down and caught the tip of Employee #1's right thumb. Employee #1 received an avulsion and crushing injury to the right thumb for which he was hospitalized.
