EMPLOYEE KILLED BY FLYING MANHOLE COVER AND MOLTEN SULFUR

On June 19, 1994, a sulfur tank truck was overloaded with liquid sulfur in the Garson Unocal Refinery by Employee #1, the driver. Later when asked by the Unocal Refinery Security to reduce the truck load to meet DOT requirements, the driver with the help of the Unocal process operator used air pressure above the internal working pressure of the tanker to reduce the load. The excessive air pressure ruptured the hatch of the tanker and Employee #1 was hit by flying hatch cover pieces and splashes of molten sulfur which killed him.
