Investigation Summary
At approximately 5:15 p.m. on April 13, 1995, Employee #1 was walking in front of a 15,000 lb tractor that had pulled into the truck yard of the Meiko Freight Facility. The truck driver had stopped momentarily to chat with a fellow truck driver. Unaware that Employee #1 was walking in front of his truck, he began moving his truck until he felt that he had run over some obstacle. He stopped and looked in the mirror and saw a person lying on the ground behind his truck. Employee #1 had been run over by the truck's back tires and dragged approximately 20 ft. Walkways were not defined. Trucks and pedestrians shared the freight yard. Employee #1 sustained multiple fractures and crushed bones in both legs and required hospitalization.
