Investigation Summary
On July 26, 1997, Employee #1 was working in the Los Angeles Metro Rail Service tunnel that was being advanced from the La Brea service shaft in Hollywood. The tunnel ground support system consisted of lattice girder ring sets and shotcrete, with a shotcrete blanket on the tunnel bench face. Employee #1 was on the right side of the face in a 2 1/2 ft deep depression about 4 ft from the bench face, hand-shoveling to prepare for the placement of invert lattice girders. The backhoe operator and the face miner on the left side noticed that the 17 ft wide by 10 ft high by 4 in. thick shotcrete slab was starting to move, and yelled out a warning. The backhoe operator attempted to use his bucket to hold back or pin the slab, but it broke apart. The piece on the right slid, tipped forward, and struck Employee #1 as he scrambled from the face area. He suffered a fractured pelvis and was extricated with the backhoe and transported to the La Brea shaft The Los Angeles Fire Department and paramedic unit administered first aid at the site before transporting Employee #1 to Cedar Sinai Hospital.
