Investigation Summary
Employee #1 was checking the cheese production line when he noticed an ice buildup on the end roller of the conveyor. Without turning off the machine, he put his hand inside the belt, next to the roller, to scrape off the ice. Either his hand slipped or he otherwise lost control of it, and it became caught by the end roller. Employee #1 sustained serious hand injuries, for which he was hospitalized.
