Investigation Summary
Employee #1 was repairing a seamer machine. He shut off the machine to adjust the guide rail, then turned the machine back on to observe its operation. He decided it was not running smoothly and gave it a final tap with a soft hammer. While holding the hammer, his little finger hung down and became caught between the infeed and the guide rail. The tip of his little finger was amputated below the nail.
