Investigation Summary
Employee #1 was obtaining testing equipment from a storage cabinet when the electric controller to CIP tank #7 (Barksdale temperature controller) malfunctioned, causing the temperature in tank #7 to rise above a specified level while steam was injecting into the tank. This resulted in a violent boiling of the water/caustic solution and subsequent splashing boilover from the tank onto Employee #1. The tank's lid was not secured to prevent it from opening and thus prevent boilover/splashing from burning Employee #1's back and legs.
