Employee's fingertip amputated in chain drive
Employee #1 was using a paper towel to clean the feed chain of an Ossid 500 stretch wrap machine while the machine was feeding trays of chicken parts. The chain caught his right hand and amputated the tip of his finger. He had not de-energized the machine before cleaning it.
