Investigation Summary
Employee #1 was servicing the oil separator in a waste water plant. He filled a 5-gallon bucket with waste oil and was carrying it to the disposal area out of the confinement berm when the bucket hit the 12 in. tall berm wall and tipped forward. Oil spilled over the ground and Employee #1 walked in it as he stepped over the wall. He slipped and fell, fracturing his right hip. Employee #1 did not lift the bucket high enough to clear the wall.
