Investigation Summary
Employee #1, of Easton Aluminum, Inc., was using a Robella D'Asti tube bender to take small kinks out of 12 5/8 in. long aluminum tubing. He was working at a normal pace, pushing the aluminum piece with his left hand through the middle of the roller system, which was comprised of one roller on top and two rollers on bottom. He was feeding the aluminum piece palm up. Since the three rollers system was 12 in. long, little extra length of tubing existed for him to hold onto. His left ring and middle fingers became caught under the aluminum tube and the first roller and were amputated at the first joint.
