Employee's finger amputated by table saw

Employee #1, of Northrop Grumman, was cutting a 17 in. by 33 in. piece of plywood through the 1/4 in. dado blade on a table saw. The board lost contact with the saw fence and became unsquare. The board bound against the rotating blade, resulting in the board being forced out of position. Employee #1's left hand was brought under the guard and into the blade. The board struck him in the stomach. Employee #1 suffered partial amputation of his left ring finger and the abrasion of the side of his left index finger.
