Employee's fingers amputated in punch press

Employee #1, of Posada Welding, Sun Valley, CA, had just begun work. When he arrived, the die of an L & J punch press had already been set, so he started to work. He was working with round flat pieces of metal approximately 3 to 4 in. in diameter. The punch press was punching the pieces into cup-like fixtures for candles. Employee #1 had completed approximately 25 pieces using a foot control pedal. He placed his left hand into the unguarded point of operation to position a piece and somehow pressed the foot control, but failed to remove his fingers. Employee #1 suffered partial amputations of his left ring and middle fingers.
