Employee's finger partially amputated by bit

At approximately 8:00 a.m. on October 28, 1994, Employee #1, of Harmon Speaker Mfg., was in the wood mill department operating a large router (wood shaping machine), equipped with a 1 1/2 inch diameter routing bit. He was finishing wood speaker mounting boards. As he turned the wood stock around the stationary router bit, his left index finger struck the revolving bit and was partially amputated.
