Investigation Summary
At approximately 9:00 a.m. on June 30, l995, Employee #1, of Vons Companies, Inc., was in the receiving area of a store, placing boxes in the baler. Employee #1's supervisor was operating a "Big Joe" lift truck and asked him to help put some wooden crates from the meat department up onto the overhead storage area. Employee #1 loaded the boxes on the forks, then climbed onto the forks in front of the boxes. The supervisor raised the forks about 10 ft to the storage area. Employee #1 was trying to position himself better on the forks when he fell and landed on the concrete floor. He suffered back injuries and was hospitalized for 3 days at Verdugo Hills Hospital. He wore a back brace and had physical therapy and follow-up doctor visits. Employee #1 returned to work in September 1995.
