Investigation Summary
Employee #1, of Anthony Manufacturing Co., Inc., was weighing large shipping crates that measured 6 ft 2 in. high by 1 ft wide by 11 ft long. He lifted one end of a crate, which weighed approximately 600 lb, so that a coworker could shove a scale underneath. When Employee #1 lifted the crate to take off the scale, the crate slipped from his hands and tipped, crushing and partially amputating the middle finger of his left hand. He was hospitalized.
