On August 17, 1995, Employee #1, of Technicolor Video Services, was adjusting the alignment on a Shanklin shrink-wrap machine (Model No. HS-3) on line 1 of the packaging floor when his right index finger was partially amputated. (The original form did not indicate how the accident occurred.)
