Employee's fingertip amputated by rod back-off tool

On October 26, 1995, Employee #1 and a coworker were installing an approximately 35 lb rod back-off tool on oil well service rig #448. Employee #1 was holding a 1 1/8 in. polish rod between the rod back-off tool and a 2 7/8 in. tubing collar. The coworker manually tightened the two bolts on the back-off tool and then reached for a wrench to further tighten the bolts. Employee #1 was moving one hand below the rod back-off tool when the tool slipped down the rod. Employee #1's left ring finger became caught between the bottom of the tool and the tubing collar, amputating the tip.
