Investigation Summary
On July 25, 1995, Employee #1 of Tommy's Designs, suffered chest pain, headache, nausea, and blurred vision associated with exposure to and inhalation of hazardous substances. Subsequent investigation revealed that there was improper storage and disposal of paper wipes contaminated with chemicals used in screening.
