Employee injured when hit by loader

At approximately 3:40 p.m. on April 23, 1996, Employee #1 was checking the humidity of the material used to grow mushrooms at a mushroom farm. He had been doing this six times a day for about two weeks, including several times on the day of the accident. The material was stored on a large concrete slab called a wharf. A large Case loader, model W14C, that was used to move materials such as large bales of straw, was also on the wharf. The loader operator did not know Employee #1 was there and struck him while backing up the loader. Employee #1 suffered a fractured hip and shoulder injuries and was taken to the hospital, where he stayed for three days. The loader was equipped with a functioning back-up alarm. No citations were issued related to this accident.
