Employee suffers burns in fire ignited by torch spark

At about 1:30 p.m. on June 30, 1993, Employee #1 was to cut apart some old metal 55 gallon drums using a plasma cutting system. He did not fill the drum with carbon dioxide, which was available, and he did not check to see if there were flammable gases in the drum, which had been sitting around for many years. He thought the drum had been used for diesel. Employee #1 took off the cap to the bung hole (a hole on the drum top about 3 in. in diameter) and reached over it to begin to cut the top of the drum. As he did so, a spark went into the drum and fire came out through the bung hole. The right side of his body and the underside of his right arm suffered 1st- and 2nd-degree burns.
