Employee's finger amputated by unsecured equipment

At about 4:30 p.m. on May 12, 1993, Employee #1 was helping to hook up a Cima sulfur duster to a Heston tractor 100/90. This was a three-point hook-up: the top and one side were hooked up and secured and the other side was on but not secured. Employee #1 had lifted the duster off the pallet by about 6 inches when it fell, catching and pinning his left ring finger between the duster's underframe and the pallet. His finger was amputated at the first joint. Employee #1's supervisor was present. Normally, employees did not try to or have to move the equipment in this type of situation. Therefore, a general citation was issued.
