Employee crushes hand in power press while adjusting screw

Employee #1 was performing adjusting/testing operations on the die area at the point of operation of a 46 ton Federal power press. The required lockout/blockout precautions were not performed. During a test cycle, Employee #1 put his hand into the press, attempting to adjust a screw that tensioned the die. He sustained a severe crushing injury to his right thumb and index finger, resulting in the partial amputation of the right thumb and a total amputation of the index finger. An accident-related citation was issued.
