Investigation Summary

Investigation Nr: 170604474
Event: 10/28/1993
Employee's small finger amputated by blade

At 9:15 on October 28, 1993, Employee #1 was working his second day at Innovative Wood Mill. He was slotting wood by the router machine when he felt a very strong pull, maybe because part of the wood was harder. He picked up his hands, and released the wood. In this rapid motion, Employee #1's finger became caught in the blade. Two joints on his small left finger were amputated and another finger was lacerated. There were no witnesses. Employee #1 went to the company office and was taken to Advantage Care Clinic. After an hour he was transported to South Bay Hand Surgery Center and then, at 2:00 p.m., taken to Torrance Memorial Hospital, where he stayed for 3 hours (to 5:00 p.m.). He was home for 3 days. To turn on the machine, Employee #1 plugs in the chord and turns the button on. The machine does not go off automatically.
