Employee's fingertip amputated when caught under pipe

Employee #1 was unloading an 8 ft section of 24 in. diameter concrete pipe by rolling it onto a bucket loader, using a crowbar as a lever. In the process, the loader operator moved the bucket forward and the pipe rolled back. Employee #1's right middle finger between caught between the crowbar and some dunnage, amputating the tip.
