Employee injured in same-level fall onto floor

On August 4, 1997, Employee #1, a research worker at a California medical center, slipped and fell to the floor as she came out of a restroom. She suffered injuries to her radial (arm) bone, and was treated at the employer's facility and sent home that same day. On August 14, 1997, she was admitted to Kaiser Permanante Hospital, where she remained until August 18, 1997. Employee #1 would not give the reason for that stay, nor was she willing to cooperate in the accident investigation. At the time this report was written, she had not yet reported back to work.
