Employee's finger amputated by shaper machine

On October 13, 1994, Employee #1, a machine operator for Innovative Wood Milling, was operating a wood shaper. The employee was manually pushing a 1/2 in. by 8 in. by 3 in. piece of wood through the shaper when it snagged, pulling the employee's fingers into the shaper and amputating the right middle finger at the first joint. Employee #1 had removed the auto feeder prior to operating the shaper. Employee #1 was transported by paramedics to Advantage Care Clinic, then to Western Hand Clinic. A violation of T8CCR 3942(a) was found.
