Employee killed when fixed wooden ladder breaks loose

At approximately 10:00 a.m. on October 25, 1995, a crew from Anderson Roofing and Sheet Metal Co. was preparing for roofing work at an apartment building in San Francisco, CA. The foreman was trying to determine an access way to the roof and asked a resident if there was another way to access the roof other than an old fixed wooden ladder on the balcony. The resident said no, explained he was late for work, and asked the foreman how long the job would take. He also stated that he needed to lock up the door and did not want roofers walking through his apartment all day to access the roof. A roofer on the crew fell to his death when a fixed wooden ladder broke loose.
