Employee's fingertip amputated between motor pulley and belt

Employee #1, of Marriott Hotel, International, had changed a fan belt and was checking the tension of the belt. His right index fingertip was amputated when he caught it between the motor pulley and the belt, which he had rotated manually. The motor pulley was 4 in. diameter, powering 5/8 in. wide dual belts to a 10 in. pulley that turned the fan shaft inside a 56 in. diameter meeting room HVAC exhaust fan duct. Employee #1 had performed lockout/blockout correctly, but he misjudged the momentum caused by manually rotating the drive belts to check tension.
