Investigation Summary

Investigation Nr: 170204481
Event: 10/13/1994
EMPLOYEES' FINGER AMPUTATED WHEN CAUGHT IN BELT AND PULLEY

At approximately 12:45 p.m. on October 13, 1994, Employee #1 was working as an apprentice stationary engineer in a baking company. A wrapping machine had stopped and could not be started. Employee #1 went to the machine and lifted a plastic cover with his left hand to stop the machine. Employee #1 then opened a set of double doors on the lower part of the machine to gain access to the drive motor. Employee #1's left arm and hand were stretched to hold the cover open (which shuts off the machine). At the same time Employee #1 used his right hand to reach down to move the drive belt. Employee #1 had just grabbed the drive belt when the wrapping machine started. His right little fingertip was caught and pinched between a belt and pulley drive and was amputated. Apparently, Employee #1's arm span was not sufficient to maintain the pressure needed on the cover to keep the machine shut off.
