Investigation Summary
On September 23, 1993, the forklift operator was operating a forklift with an extendable boom to deliver a steel beam to Employee #1, the connector. The beam was attached to one fork of the forklift by a C-clamp. The C-clamp slipped off the fork and fell on Employee #1 (approximately 14 ft above grade), amputating his left ear and arm. Employee #1 then fell 14 ft.
