Investigation Summary
Employee #1, of Stewart Construction Company, was bolting together four metal I-beams to form a roof rafter. The I-beams were positioned on 4 by 4s on a concrete floor. As Employee #1 was walking beside the I-beams, one of the supporting 4 by 4s failed, allowing a beam to roll over on to Employee #1's left leg. He suffered crushing injuries to the lower portion of his leg and was hospitalized.
