Employee's hand injured when caught between rollers

Employee #1 was cleaning the in-rolls of a 24 in. rubber mill when his glove became caught between the rolls, which were set at 1/4 in. He was hospitalized with fractured fingers. The rolls were about 50 in. off the standing surface. The nip point was about 25 in. inward from the mill's toeboard. Because the mill is an older model, it is not equipped with point of operation guards, although an overhead trip rod was available for emergency stopping. Based on an estimated circumference of 37.68 in. (12 in. diameter), at 26 rpm, a surface travel distance of just over 14 in. is permitted. During the investigation, three tests were observed with surface travel distances of between 9 and 12 in. before stopping. Reaction time was not included and is not required by regulations. Additionally, the trip rod was incorrectly installed about 3 in. toward the operator.
