Employee falls from three-story scaffold

At about 12:01 p.m. on May 1, 1992, Employee #1, a 41-year-old carpenter, fell from a three-story scaffold he had built to install 4-foot by 8-foot sheer panels on the exterior of a building. Employee #1 was conscious after the fall and did not complain of pain or loss of breath. He was transported to a local treatment clinic and released. Employee #1 was working as an independent contractor. The scaffolding consisted of a single, narrow platform without guardrails.
