Investigation Summary

At approximately 10:40 a.m. on July 23, 1992, Employee #1 was supervising the removal of bridge falsework. An I-beam became unbalanced, slid off a lower support beam, struck his left leg, and pinned his right leg against the concrete bridge railing. His right leg was amputated at mid-calf, and he sustained a fractured right femur and a crushed left femur. Another supervisor directed a crane to the location and, with 2 coworkers, placed the cable hook on the beam and lifted it off Employee #1. A superintendent called 911 and first aid was administered by coworkers and a passing off duty paramedic until the ambulance arrived. A life flight helicopter transported Employee #1 to a local hospital.
