Employee's Hand Caught in Machine

An employee, working at a wine bottle filling and case packer line, tried to remove a box case that was stuck in the bottle casing machine. He did not deenergize and lockout/tagout the machine before putting his hand on the conveyor belt. The pulley caught his right index finger and cut it. He was taken to the hospital for treatment and returned to work after a few weeks.
