Investigation Summary
On January 21, 2005, Employee #1 was working for a stucco contractor and was assigned to start taping and covering the facia board on the new house, to protect it from color application. Employee #1 was working from a 4-ft-high A-frame scaffold. Employee #1 was looking up while working and walking along the scaffold and he walked off of the end of the scaffold plank. Employee #1 landed on his left foot and injured it. Employee #1 was hospitalized.
