Investigation Summary
On September 10, 2003, an employee was assigned to monitor the Sinclair labeler. Sinclair labeler was mounted on the sizer machine in the Plant Number 4. The labeler jammed up with labels on the end head; the employee tried to clear the head with her hand. She did not de-energize the label machine. The machine cycled and crushed her hand. She was transported to the Sierra Kings District Hospital for treatment. She was hospitalized and treated for fracture.
