Employee dies when crushed between forklift and trailer

On December 19, 2003, Employee #1 was helping to bring a semi-trailer through the front door of a shop for repair. The street in front of the business had a high crown in the middle of the street. The driver was backing-up the trailer and it was hanging up on the curb edge. Employee #1 tied a chain to the back of the trailer and was pulling on it with a forklift. The chain fell off the forklift and when the employee got off the forklift to replace the chain, another employee yelled at the semi-trailer driver to stop. The driver thought he said go and accelerated the truck and Employee #1 was caught between the trailer and forklift. Employee #1 was transported to UMC Hospital in Fresno, CA where he died.
