Investigation Summary
On March 9, 2001, Employee #1, a foreman, and coworkers were in a field being farmed. A tractor and trailer was used through the furrows of the field to harvest the spring mix. Employee #1 was walking next to the trailer when slick conditions caused him to slip. His foot was run over by the trailer and fractured. Employee #1 was hospitalized.
