Investigation Summary
On November 21, 2001, Employee #1, a laborer, was cleaning machinery after the harvest season, blowing dirt and debris off of the machinery. Employee #1 accessed some overhead duct work by climbing on top of the equipment and then onto the duct work to blow dirt off. He traversed approximately 30 feet of duct work and fell 20 feet to the concrete floor. He fractured some bones and had a severe head injury. He was taken to UMC in Fresno, California to treat his fractures and head.
