At approximately 8:30 a.m. on September 30, 1999, Employee #1, of Custom Commercial Millwork, Inc., was using an unguarded table saw to cut plywood when his fingers contacted the saw blade. He suffered a partial amputation of three fingers on his left hand and was admitted to Lancaster Community Hospital in Lancaster, CA.
